For your application to the Dental Hygiene program to be considered complete it must meet the following requirements.

1. Be complete
2. Including two sets of official sealed transcripts from all colleges and universities attended
3. Include 1 passport sized photo of yourself for identification purposes only.
4. Mailed via certified mail, return receipt. Applications will not be accepted in person. No exceptions.
