WANT TO CONTINUE YOUR SERVICES AFTER YOU
TRANSFER TO A FOUR-YEAR COLLEGE OR UNIVERSITY?
Call or visit the Disability Resource Program at the school
you’re transferring to.
Ask them what documents they need.
Ask them how soon
they need them.
Ask them how we should send them (fax?
mail? other?).
If you have a learning disability or ADHD, bring the above
information
in writing (yours or theirs) to
the Learning Skills Program (LSP), Room 1073.
If you have a physical or other disability, bring the above
information
in writing (yours or theirs) to
Disabled Student Programs and Services (DSPS), Room 810.
Sign a release form for the LSP or DSPS that will allow us
to share your information with your new school.
Give us at least two weeks
to get the documents ready.
Please remember we have very limited
staff in the Winter & Summer semesters, so get your request
in during the Fall or Spring semester.
