How to Become A Student at the Center

Step 1 - Call the Center: 831-425-0622

You or a friend or family member are welcome to call the Center to begin the process of admission. We will be happy to answer any questions you might have. We will ask a few questions too, that will help you or your loved one decide if the Center is the best place for you to continue your rehabilitation. The following information will be helpful:

  • Your doctor's name and phone number.
  • Your diagnosis or the date of your stroke.
  • Whether or not you are on MediCAL (not MediCARE).
  • Whether or not you are receive SSI--Supplemental Security Income (this is not the same as Social Security).

Step 2 - Attend a one-hour initial consultation with one of our counselors.

After your initial contact with the Center, a counselor will call to set up a one-hour initial consultation. During this time, you will tour the center and explore your options for participation. If you wish, you may download the following forms, print and complete them and bring them filled or partially completed to your consultation. Some of these forms are easier to complete at home where you have a list of your medications and your personal records. There is a $27.50 tuition fee for this counseling session, and whether or not you start classes, you will receive 1/2 unit of Cabrillo College credit.

Step 3 - Attend a full day evaluation and set your rehabilitation goals.

Your final step before admission will be to attend a full day at the Center, for an instructional/therapy evaluation with staff members from each of our four departments. You will be staying for lunch also, so you will have an opportunity to talk with other students about the program and their classes.

For the evaluation day, you need to bring any forms that are not completed plus a brown bag lunch.

The following is a sample evaluation day:

  • Arrive at the Center at 9:30.
  • Fifty minute speech and language evaluation.
  • 10 minute break
  • Fifty minute physical therapy evaluation
  • 10 minute break
  • Fifty minute occupational therapy evaluation
  • Lunch
  • Wrap up with counselor to set goals and create your Individualized Educational Plan based on the recommendations of the evaluating team and your expressed interests.
