| Transcripts
You must request official transcripts in writing. Unofficial transcripts
must be picked up in person (only for academic records
from 1982 to present). For records prior to 1982, students
must submit a request for an official transcript. A picture
ID is required to pick up an official or unofficial transcript. If you are requesting or picking up transcripts on behalf of someone
else, you need authorization via a note signed by the student.
1. You may download the Transcript
Request form in pdf format, fill it out and return with payment
to Cabrillo College.
2. Or, you may provide the following
personal information in writing and mail or fax to the address below.
a)
Name or names used while attending Cabrillo College
b) Social Security Number or Cabrillo ID number for students registered
Fall 1999 to the present
c) Date of birth
d) Approximate dates you attended Cabrillo College
e) How many copies of transcripts are needed
f) Location and address where transcripts are to be sent
g) Current telephone number with area code
h) Signature on request form
Transcript Fees
The first two copies ever requested are free. After that, all transcripts cost $3 per copy.
There is an additional fee for rush transcripts.

Payments can be made
by check, money order, or credit card (Visa, or Master card only).

date and a signature.

Final Transcripts
Prior to Transfer
In your last semester at Cabrillo, once you have determined where
you will transfer, you may request your final Cabrillo transcript
and certification of your General Education (IGETC or CSU General
Education) by submitting the “Transcript Request” Certification
Form to the Admissions and Records Office at Aptos or the Watsonville
Center.
