Certificate of Proficiency 3.2.2

 

The catalog details courses that are required for these certificates. Use the "Petition for Certificate of Proficiency" form.

Under "required course/elective number & title", list only those courses that are required by the certificate. List units attempted, units completed, grade, and grade points for each class. If the class is in-progress, list the units in the appropriate column.

In the unit summary section, list only the units attempted, units completed and grade points that are from courses relevant to the Certificate of Proficiency.

The GPA to date is calculated only on those courses relevant to the Certificate of Proficiency. An average of 2.0 GPA is required in courses specific to the Certificate of Proficiency.
