Re-entry/Dismissal Petition 3.5

Counselors should be familiar with the section of the catalog that details our policy on Probation and Dismissal. It is listed in the catalog under "Dismissal". Often, if a student is being blocked from registration, they may be on either academic or progress dismissal. To verify if a student is on some form of dismissal, go to the PERC screen and enter the student ID or name. If it lists Academic or Progress Dismissal, and there is no end date listed, the student is on dismissal.

Counselors are a part of the re-entry process for dismissed students. We help the student identify issues that may have contributed to their earlier lack of success, and then identify what interventions can be implemented to improve the situation. Counselors can make recommendations such as tutoring, follow-up counseling appointments, a reduced units load, or study skills classes.

As a part of the re-entry process for dismissed students, we use the "Re-entry / Dismissal Petition" form, as follows:

A. For students who are not currently enrolled and have been out of Cabrillo for one or more semesters, Counselor approval is all that is needed for re-entry:

              Student fills out Part 1 of the new petition.

              Counselors fill out Part 2 of the new petition.

              If you do recommend the student be readmitted, you must clear their restriction in Datatel by going to the PERC screen (follow instructions below).

              Sign the petition, give the yellow carbon copy to the student, and give the original to A & R. If you made counselor notes in the student’s file or in the PERC screen you can attach those to the original.
               

          B. For students who have not been out for one or more semester, Counselor approval, and approval from Gloria is needed:
              Have the student fill out Part 1 of the new petition.

              Counselors fill out Part 2 of the new petition.

              Send the petition (original and the yellow copy) to Gloria in Admission and Records.

              Once Gloria gives final approval, A & R will clear them in the PERC screen and they will notify the student.

C. This procedure will need to be modified during registration periods when the student needs immediate approval (meaning counselors may need to clear dismissals even for those continuing students).
PERC instructions:
            1. Type PERC and Enter
            2. Type Student’s Name, SS3, or ID# and Enter
            3. Hit F5 to get to the window of the dismissal that is active. The active dismissal is the one that has no end date. When you put an end date, it removes the student from dismissal
            4. Once you’re on the dismissal that is active, hit return 3 times to get to "end date".
            5. Type today’s date.
            6. (OPTIONAL – you can hit F2 to detail in to a comment screen if you want to make notes).
            7. F9 and Enter to finish.
