Please print all forms on white paper. If you have trouble printing or do not have access to a printer, these forms are also available to pick up at our office.
Stafford Loan Request - REQUIRED
- Please print on white paper and complete with blue or black ink.
- Write legibly and initial any crossouts.
Loan Disclosure and Terms - REQUIRED
- Please read carefully, print, sign and date in ink.
Worksheet for Student Borrowers - REQUIRED
- Answer all 10 questions. Use referneces provided on worksheet to research answers. We want you to get an accurate idea of your ability to repay your loan.
Budget Planning Worksheet - REQUIRED
- Use realistic estimates - We are not judging how much you spend. We want you to get an accurate idea of how much you actually need to borrow.
- Visit www.edwise.org for help in completing an accurate Budget Plan.
Student Loan Quiz - REQUIRED
- Quiz answers can be found in Student Loan Guide and were also part of your Entrance Exam.
Late Loan Form - REQUIRED
- This form must be submitted with your loan packet if your applying in the Fall but it is after the Fall loan deadline.
