A physical exam is required prior to entering your program. The exam must be 6 months or less prior to start of program. All alternates must complete this as well.
You must take the form to your healthcare provider to fill out. This is the form that you will be uploading to your online tracking account. This is the ONLY form that you can use, no other physical exam form will be accepted.
If you are an alternate do not complete the following until officially accepted by your program:
- Drug Screen
- Compliance fee
- Background Check
- COM
- Mask fit (Nursing only)
